What to Bring to Your Registration Day

During your Registration Day, we will collect and make sure all your necessary paperwork is complete. Please review the following requirements in order to prepare for your Registration Day:


1.) Photo and Photo ID: If all paperwork is complete you will be required to take a photo for your volunteer profile, so please plan to look presentable. We will also very your identify utilizing a photo ID (government-issued IDs preferred; school IDs may be accepted).


2.) Role / Location Preference and Availability: Please keep in mind our currently open volunteer position and come prepared to set your regular weekly schedule during your Registration Day. Volunteer placement is based on the Clinic’s needs, so please have a few day and time options as well as a desired start date in mind to ensure a better chance of securing a regular schedule with Venice Family Clinic.  3.) Mandatory Paperwork

For ALL VFC Volunteers:

Mandatory Depending on Your Role:

Immunization History: If your role involved direct patient contact, you will need to collect your immunization history and submit on your first day:

  • Medical documentation of TWO MMR vaccinations OR Medical documentation of ONE BLOOD TITER indicating immunity to MMR
  • Medical documentation of a negative TB skin test completed within the last 12 months OR a negative TB blood test OR a negative TB chest x-ray completed within the 3 years

Location:  Simms/Mann Health and Wellness Center, 2509 Pico Blvd. Santa Monica, CA 90405. Parking is free on our lot. The Santa Monica Blue Bus Line 7 stops (Pico Blvd. & 26th St.) in front of our clinic for your convenience. When you come in, tell the security desk you are here to meet with the Volunteer Department.


NOTE: We are unable to accommodate any latecomers, so please plan to be on site AT LEAST 15 minutes before your scheduled time. If you are late or miss your Registration Day, you may not be allowed to continue in the application process.
